The Registered Dietitian Nutritionist (RDN) serves as a member of the interdisciplinary care team in the treatment of patients. Provides high-quality education, counseling and support to the patients and community that he/she serves. Applies in-depth knowledge and skills in nutrition, biological and social sciences, communication, counseling, and education to provide medical nutrition therapy, as well as diabetes self-management education, training and support.



  • Oversees day-to-day functions of nutrition counseling practice and diabetes self- management programs at specific ACP office location(s). Including the following tasks:

    • Provides medical nutrition therapy (MNT) for patients of all age ranges for a variety of nutrition-related health conditions.

    • Provides diabetes self-management education and training (DSMET) for diabetes patient population.

    • Provides patient/family counseling on appropriate nutrition recommendations and develops/implements plan of care. Follows up via phone and office visits.

    • Assesses nutritional status of patient population using anthropometric measurements, laboratory indices, nutrition and social histories. Identifies and labels the nutrition problem that the patient has that the RDN is responsible for treating independently.  Uses a PES Statement to communicate the nutrition diagnosis (problem, etiology, and signs/symptoms) in the patient chart.

    • Counsels patients on an individual basis regarding nutrition and behavior changes for desired outcomes.

    • Conducts group sessions for weight management and diabetes self-management education, including education on the planning and preparation of food at home for these patients.

  • Collaborates with medical office clinical lead/medical director, Practice Administrator and support staff while providing both individualized and group counseling for patients. Coordinates with support staff and supporting departments to remind patients of appointments and collects appropriate information (i.e. insurance) prior to appointments. Performs patient outreach to help meet HEDIS measures. Coordinates with supporting departments (i.e. referral) on nutrition services covered by insurance and performs patient outreach when applicable.

  • Develops nutrition education materials including meal and snack ideas and recipes to be added to group's resources following standards of practice. (provides translation if possible)

  • Records nutritional assessment/plan summary for medical record. Maintain accurate and complete medical charts and supply of educational materials.

  • Participates in ongoing training and orientation on E.H.R. system, medical home requirements, compliance, continuing education and team building. Maintains expert level knowledge on current research in nutrition and health. Provides training for staff and physicians when appropriate as needed. Completes all necessary chart notes in an accurate and timely manner. Performs other job-related duties as required.

  • Performs other job-related duties as required.


  • Bachelor’s Degree and/or Masters’ Degree in nutrition/dietetics

  • Successful completion of Commission on Dietetics Registration (CDR) examination for Registered Dietitian/Nutritionist (RD/RDN) credential

  • Certified Dietitian/Nutritionist by the State of New York         

  • Successful completion of the National Certifying Board of Diabetes Educators (NCBDE) examination for Certified Diabetes Educator (CDE) credential Preferred

  • Three years as a registered dietitian nutritionist required

  • Knowledge of nutritional/dietetic principles and applications and the use of nutrition care process required

  • Knowledge of concepts and methods of patient education including teaching and educational aids. Effectively develops and conducts education programs, serves as a resource to care teams required

  • Knowledge of how to design and implement special diets specific to a health condition while acknowledging different cultures and religious beliefs required

  • Skill in nutritional principles through appropriate assessment of status/needs and development of care plans based on appropriate calorie levels and nutrient adequacy required

  • Skill in understanding of nutritional impact on health status by developing appropriate diets for patients with diabetes, hypertension, hypercholesterolemia, adult and pediatric obesity, pre/post natal, renal disease, gestational diabetes, cancer, digestive disorders, vitamin/mineral deficiencies etc. required

  • Ensures that all patients receive personalized, prompt attention and are treated with receptiveness, dignity and respect. Recognizes and performs duties required independently, even when not directly assigned. Attends all meetings as directed by Director of Nutrition and Medical Director.

  • Demonstrates good judgment, discretion and ability to problem solve. Demonstrates flexibility in the acceptance and completion of work assignments required

  • Two years of experience in a health care setting preferred
