Qualifications:


• Bachelor’s Degree (R)
• Master’s Degree in Social Work (R)
• New York State Social Work License - LMSW (to work under qualified supervision as required by NYSED) (R).

• Minimum two to three years’ experience in social work with background in behavioral health with diverse patient populations (i.e., HIV, homeless, mentally ill, elderly, children adolescents, etc.). (R)
• Technical knowledge of medical social work and relevant community resources, and the ability to develop linkage with community organizations (R)
• Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions, under the supervision of a licensed provider. (R)
• Ability to, in collaboration with the department, provide input to the design and implement clinical pathways and protocols for treatment of selected chronic mental conditions (R)
• Skill in proper use of tools such as the Diagnostics & Statistical Manual (DSM V) for identifying clinical symptoms (R)
• Should be comfortable with the pace of primary care, working with an interdisciplinary team and have strong communication and human relation skills (R)
• Knowledge of the development and maintenance of information/referral systems (R)
• Healthcare and mental health experience (R)
